Pass** - some chemical spray bottles were not labeled to disclose contents to prevent potentially hazardous accidental mixing of chemicals.
** - many high-risk foods such as meats and cut vegetables, ready-to-eat foods were stored well above the “fill line” of the counter coolers. The meat was measured (exterior reading) of 10’C. Do not stored high-risk foods above the fill lines
** - staff stored 5 pieces of salmon at room temperature (thaw?). PHI directed for the salmon to be put away in the cooler. When the PHI was leaving, PHI observed what appeared to be seafood improperly thawing at room temperature in a sink, within a bucket of stagnant water. The water was not on or running. Retrain staff on safe thawing methods.
** - scoop was stored with the dirty high-touch surface of the handle in direct contact with food ingredients (flour)
April 17, 2026: Repleta fly population observed in the bar area. Education on Repleta fly abatement was provided onsite. , , July 30, 2025: Small black flies still observed on the walls by the glasswasher and the dining booths close to this area., , Feb 12, 2025: Small black flies still observed on the walls by the glasswasher and dining booths close to this area., , Nov 28, 2024: Small black flies still noted in the bar area., , Oct 1, 2024: Numerous small black flies are noted in the bar area as well as in the dishwash area of the kitchen., - Pest control contract is in place. Implement abatement measures.
** - cold water was leaking from the dishwasher sink faucet. Please repair
** - unfinished wood walls were observed in back hallway. Recommendation to paint/seal as it could be contributing to fly problem. Discussed onsite. All surfaces in a food facility storage and processing areas should be smooth, cleanable, non-absorbent and durable.
** - poor sanitation was observed in the kitchen the bar that includes but is not limited to: the interior fan diffusers of the counter coolers, the freezer in the bar, the upside down glass washers, all containers/buspans, the grill shelf, the underside of the grill shelf, the spice containers, hanging utensil containers, all spray bottles, potato fry press, shelving below food equipment. Two metal stainless steel bowls were observed black with soot, grease, and proper cleaning. The burger press was filthy. The probe thermometer was stored in filth and grease.
** - fryer basket was in poor condition exhibiting broken metal/missing pieces. Please replace.
** - poor sanitation was observed in the washrooms such as: the toilet bases in the washrooms, the wall in the staff washroom. , ** - bar soap was observed in the staff washroom: please use only liquid soap.
** - poor sanitation was observed in the kitchen the bar that includes but is not limited to: the bar floors under equipment and in hard-to-reach places (broken glass in many areas as well), the interior of the glasswasher, the door/door frame for the walk-in cooler, walk in cooler fan, dishwasher area walls, all faucets for sinks, dishwasher controls, handwashing sink area of the kitchen (walls), cupboards across from the handwashing sink (kitchen)
