1. Used knife was stored in crevice between prep coolers. This is not a sanitary way to store knives as crevice is not cleaned regularly., , 2. A plastic cup was used as a scoop in spice mix. Either clean and sanitize cup between uses or use scoop with handle to ensure bulk food is not contaminated.
