The bleach sanitizer in the kitchen was initially measured at 0 ppm. The operator was instructed to prepare a fresh sanitizer solution, which was subsequently measured at 100 ppm.
The rear door screen was observed to be torn and damaged, and was propped open with a stopper, creating a potential entry point for pests. The operator was advised to repair or replace the screen to prevent pest entry. Operator closed the door upon request.

They had received a notice that the water would be shut off to the building for repairs., Discussed that the proposal of only serving take out was not acceptable that with the inadequate water the facility was to be closed. Operator turned off the open sign., Left the PHI cell number for a call once the water was back on.
