At time of inspection:, , 1. Some floor tiles (close to the main exit door (north-east) were cracked and thus can pose a potential tripping hazard. There were also some flooring issues where a long piece of tape was installed (i.e. within one of the aisles)., , 2. The metal plate that is situated at the exit doors (north-east side) is in disrepair and requires repair / replacement (as this is also considered a tripping hazard as this plate has lifted and is not flush with the original flooring)., , The owner / operator of this facility is requested to conduct the following, namely:, , 1. IMMEDIATELY generate a work order to repair any floor tiles that are cracked or pose a potential tripping hazard. , *****THIS ISSUE REMAINS EXISTING FROM THE LAST ROUTINE INSPECTION.
