Non conformeThere is no door between the living quarters and the food handling area. Options were discussed with the operator that would allow for sufficient separation.
The test strips for chlorine were expired.
Pest control checklist had not been filled out. Please see attached, print off, and complete monthly while facility is in use.
Operator was unable to demonstrate completion of an approved Food Safety Certification course. Please see attached list. Should you meet this requirement, kindly send certificate to PHI. If not, please register and complete a course of your choosing.
Metal shelves throughout are becoming worn and absorbent. They must be refinished so as to be smooth, impervious to moisture and easy to clean.
The flooring finish near the dishwasher has worn away and unfinished floor is exposed.
Hood ventilation has not been serviced professionally.
