The items were promptly removed by staff and the cutting board was removed for cleaning. , , Ensure all personal items not associated with food preparation are stored in a separate, dedicated area where it will not contaminate any food/equipment.
The guard for the meat slicer had a build-up of food debris. The operator promptly cleaned it.

The wiping cloth was placed in a QUATs solution that tested 0ppm. when told the manager made a fresh solution that tested 200ppm.
1- The flour and sugar scoops were placed directly on top of the shelf beside their containers. The scoop could get contaminated with dirt and other debris found on the shelf. When told, the manager removed for proper washing and to place them in a proper container., , 2- The bacon was left uncovered in the walk-in cooler. When told the manager put a wrapping film on top of the container., , 3- Ice was placed in a black garbage bag in the freezer. garbage bags are not food grade containers for food to be placed in. When told the manager said that their ice machine was broken and asked one of the operators to get rid of the ice in the garbage bags., , 4- The Flour bag was placed on the floor. When told the manager put it on the shelf., ,

There were scoops left inside the bulk food containers and the handles the scoops were sitting inside the containers toughing the food.
There were approximately 10 customer seats that were ripped and had holes in the back part of the seat.
There were scoops inside the bulk food bins in the dry storage area.
The ceiling on top of the cold holding unit where they prepare salads and sandwiches is showing accumulation of dust and debris that could potentially contaminate the food being prepared in this area.
