February 11, 2026: Two live cockroaches observed inside of the upright 4 door stainless steel freezer next to the central prep table. , , July 3, 2024: Live cockroach activity noted in the underside cupboards of the central prep table, as well as underneath the glass door cooler along the North wall of the facility. , , , Mar. 7, 2024: Pest control has been in to perform treatment. Some cockroaches were found inside the central storage unit, as well as in the area of the two-compartment sink. Advised the operator to notify the pest control company of this noted activity when they come to perform the next treatment., , Feb. 23, 2024: Several cockroaches were noted on some of the glue boards that have been placed out for monitoring purposes by the pest control company. One small, live cockroach was observed crawling on a freezer in the hallway. , You must contact your pest control company immediately to arrange for an on-site assessment in order to develop a plan for treatment and further monitoring of the pest issue. You are to provide documentation from the pest control company to our office by the end of next week., , , January 24, 2024: a few live roaches were noted crawling beneath the sink area. Several roaches were noted on various glue boards in the facility. Advised Rose to contact the pest control company and request they return to assess the situation and initiate treatment as deemed necessary., , Dec. 20, 2023: A few cockroaches were noted on the floor underneath/in behind the cooler against the North-West wall/corner of the facility. Two cockroaches were noted crawling inside a storage cabinet in the centre of the kitchen, across from the cooler mentioned above., Pest control records are not being maintained., You are required to have a licensed pest control company come to the facility in order to assess the pest issue and provide treatments as they deem necessary. Records/reports of each visit must be collected and provided to PHI.

The light fixture in the walk-in freezer is broken and requires repair to ensure adequate lighting.

Several light fixtures in the hallway leading to the back door are missing a shatter-proof guard. Shatter-proof guards are required over these lights in order to contain glass shards in the event that a light bulb shatters.
