Hands were not washed between a source of contamination (cellphone) and putting on gloves for food preparation. Discussed with staff. If hands are contaminated the outside of the glove will get contaminated when putting them on. Corrected during inspection.

The spray bottle of sanitizer had the label on it for a different sanitizer. Ensure all containers of chemical are properly labeled with their contents. Corrected during inspection.

There was a box of take-out containers on the floor. To be stored up off the floor. Corrected during inspection.
