** - pesticides such as "moth balls" were repackaged onsite and stored with foods near an area the staff spend time. No labels of any kind existed on the packaging. The odor was overwhelming. NEVER repackage pesticides at any time. Pesticides must be stored in the original packaging that contains safety information, cautionary statements (ie. poisonous), and chemical information. Immediately CEASE and DESIST.
** - Items that may contaminate food were stored with food products on the sales floor: a child's bike, glass fluorescent bulbs, ** - while PHI was onsite, a child was located behind the till. PHI observed the child placing dirty feet and socks on the counter where the hot food was stored. Immediately cease and desist, clean and sanitize the counters where food is stored or processed for sale
** - Food Handling Permit was not posted in public view as required.
** - hot food handling was observed which was against the Food Handling Permit restrictions which clearly state: "Pre-packaged foods and/or whole produce only – no preparation of food". ** - dirty cardboard and newspaper was observed stored with food and lining shelves. Newspaper is not food-grade. All surfaces in a food facility and storage must be smooth, cleanable, non-absorbent and durable.
** - Walk-in cooler measured ~2˚C. There was a missing ceiling tile in the walk-in cooler. The walls of the walk-in cooler seemed to be in disrepair. Please repair the ceiling and walls of the walk-in cooler to have a smooth, durable and easily cleanable surface.
** - the door to the sink room, the hot holding cabinet, and any doors or areas of high-touch were coved in grime and handprints - thorough cleaning required.
