*Opened bags of cereal were found in the cupboards. Ensure open food items are properly stored in containers with lids.
*Two plates of breakfast foods were found at room temperature and being saved for residents. Reviewed storage requirements for saving food products with management. Please review with all staff members. Food products were discarded.
*Residents’ personal food items were stored in coolers and freezers in the serveries. Dedicate and label specific areas in the coolers and freezers for residents’ personal food items. Items should be labeled with resident name or room number.
*Gaps noted around plumbing noted under the sinks in the serveries. Seal gaps and holes to prevent pest entry.

Ensure all parts of the tables are included in routine cleaning and sanitizing., *Black dust/debris noted on the ceiling areas right next to the ventilation vents. Please clean area and ensure ventilation is on good working condition.

Food for trays observed stored at room temperature. Reviewed ensuring that trays are dispensed as delivered and not dispensed extended periods of time prior to service. Keep items in steam table. , Ensure that food items are stored directly in steam table and cold items are stored in a manner that keeps them cold or stored in fridge until service (eg. dessert).

*Staff personal beverage was found in a cupboard the north servery. Remove and ensure staff personal items are stored in staff designated area, review with all staff. Corrected on site: beverage was removed by care staff.

Reclean cups and ensure cups are in clean/good working condition., , *Paper place mats were being used as liners in cupboards in the north and south serveries. Paper place mats were no longer in good condition. Replace paper mats with cleanable liner.
Thermometer not located in the 3rd floor servery kitchen. Thermometer moved from freezer temporarily. Replace thermometer.
Freezer in North Memory Care unit verified at 8 C, food products inside were melted. Ice cream, ice cream sandwiches, and frozen pretzels discarded during inspection. Ensure that cold storage units are able to maintain temperature. Repair, replace, or remove freezer.
Soap is not available at hand sink in South memory care servery and paper towel dispenser empty at 3rd floor servery. Ensure hand sinks are supplied with soap and paper towel.
