*6100- a plate of toast and eggs were found in the microwave. Discard. Review with staff members how to properly save food products for residents. Food products should be covered, dated and placed into the cooler or kept in hot holding. Food was discarded.
*3100- Residents personal food items were stored in facility refrigerator. Relocate resident food items to resident designated refrigerator. Please remind staff about the residents designated refrigerator on this unit. Completed on site.

Site was unsure when traps were placed in the cupboard. Ensure traps are monitored and full traps are discarded. Discard trap and review with staff.
*1100- Spilled cereal noted in cupboard. Clean out cupboard. , , *2300- Food splatter noted in microwave. Clean microwave , , *Multiple serveries - Coffee was being made on the units; it was found the coffee pots were not being sanitized. Staff were only washing pots in the sink. Change practice and ensure all coffee pots are adequately cleaned and sanitized. Manager instructed staff to stop using the coffee pots and call down to the kitchen for coffee for lunch. , , *Multiple serveries - Single use utensils were improperly stored. Investigate and determine if that much foam is needed.
D10 solution in bistro verified at 0 ppm. Ensure bistro is supplied with a sanitizer and disinfectant that are maintained at manufacturer's recommended concentrations. Replace sanitizer.
Fridge in unit 1100 verified at 10.3 C and unit 3100 verified at 5 C. , Thermometer could not be located in unit 1100. , Ensure cold storage unit is maintained at 4.0 C. Ensure all cold storage units are equipped with thermometers.
Mouse dropping observed in cabinet near sink in bistro. Live trap observed on bottom shelf directly in front of sink. Mouse trap moved against wall. Ensure Certified Pest Control Operator continue to assess and treat as needed. Continue to monitor. , Food products behind counter have been placed inside containers. Observed large hole in wall inside cabinet. Cockroaches identified in 1300 wing kitchen. Reviewed ensuring the following: food debris and equipment are cleaned after each use. Wipe down and clean coffee dispenser to remove spills/build up. , March 28, 2023 - Cockroaches identified in 6100 & 7100 wing serveries., , January 11, 2024 - cockroaches identified in 600/7100, 1300 and 2300 serveries.
Countertop in 1300, 2100, & 2300 servery are damaged, cracked, or lifting. Ensure food contact surfaces including counters are maintained in good repair, smooth, & easy to clean.
Cupboard doors and cupboard base have been installed in areas where dishwashers, were previously installed. Exterior of cabinet doors in laminated. Inside of cabinet door is unfinished.
