The operator was instructed to switch to single-use dishes until the dishwasher is functioning properly.

The operator informed the PHI that they do not intend to continue using the cooler., , , As the undercounter cooler is no longer in use and not intended for future use, please remove the non-functional unit from the facility. All high-risk foods requiring refrigeration during preparation must be kept away from the temperature danger zone at or below 4°C., , , ***Previous violation***, , The under-counter cooler located on the service line containing condiments was tested using an infrared and probe thermometer with both measuring at 8C. Cooler temperatures must maintain temperatures of 4C and below to inhibit the growth of harmful bacteria., , Please adjust or repair the under-counter cooler to ensure it maintains temperatures of 4C and below.
***Update April-14-25***, , Mice droppings were found in several areas, including the stairs, along the corners of the wall beneath the two-compartment sink, around the back door, the mop room, and the basement. , , Please clean the mice droppings and continue to monitor for pest activity. Identify and seal any entry points. If this continues to be an ongoing problem the services of a professional pest control company will be required to abate the pest issue., , ***Previous violation***, , Pest activity was observed in the following areas including, but not limited to:, , - Along the staircase, - Area underneath the staircase, - Area by the back door, - The mop room, - In the basement, behind the clothing racks, , Please ensure that the mouse droppings are carefully cleaned by spraying them with a 1:9 chlorine solution and allowing it to sit for five minutes. Use appropriate personal protective equipment (PPE), including gloves and a mask, to minimize exposure to potential contaminants. Locate any holes and gaps and fill them with steel wool, then seal them to prevent the entry of unwanted pests. The operator is currently responsible for their own pest control. Please monitor the pest activity and adjust the frequency of treatment accordingly.
