The floor surfacing throughout the store and most prominent in the back area of the store was in disrepair, sections were held together using duct tape, **Repair/Replace flooring. Ensure all surfaces in the food facility is smooth, cleanable and in good repair., , 4. The staff washroom opens up to an area where produce sorting is now occurring. Washroom door was left ajar, **Install a self-closing device on the door to ensure door is kept closed at all times.
