A sanitizer bucket was not prepared for the cook line, and multiple rags were observed sitting out in use. The sanitizer bucket was filled with quat and all rags were submerged at the request of the PHI.
Chlorine-based mechanical dishwasher had dish soap and rinse agent only connected and there was no chlorine-based sanitizer available for the dishwasher. Employee Chris stated they must have ran out. Two-compartment sink was set up with bleach at 100 ppm, and employees were advised that dishes could be washed in the machine, but must then be fully submerged in the sink for 2 minutes, then air dried. Sanitizer must be available by the following day.

The iodine and chlorine test strips were observed to be expired. Obtain new iodine and chlorine test strips.
Some flies were still noted throughout the bar area. Ensure regular cleaning and sanitizing is completed and pest monitoring is done.
The back door was in disrepair. The manager on site stated that a delivery driver backed into the door during delivery. Ensure the door is repaired and there is no gap to prevent pest entry.
Cleaning was mostly completed. Some further cleaning was required on the lights located to the left of the exhaust hood.
Hair nets were observed to be stored in the staff washroom. The hair nets were removed immediately upon request.

Pest control records are not available. Ensure monthly pest monitoring is completed and records are kept easily accessible.
Many flies were noted on the sticky trap located in the cupboard underneath the beer taps. Some flies were noted throughout the facility. Ensure standing water/liquid is addressed and discarded throughout the facility. Contact a pest control company for service.
The flooring was lifted at the entrance of the walk-in cooler. Repair the flooring such that it is smooth, durable, and impervious to moisture to facilitate ease of cleaning.
The drainage for the beer taps was observed to be leaking. A bucket was used to catch the drainage inside the cupboard resulting in standing water/liquid. Repair the drainage and ensure there is no standing water/liquid., , Staff was instructed to empty the bucket regularly and ensure no standing liquid is left in the cupboard overnight.
This violation has been carried over from the previous inspection report: , Cleaning is required for the following: floors with special attention to back entry and ceilings with special attention to areas near ventilation., , Cleaning is also required in the walk-in beer cooler, exhaust canopy filters, underneath the cooking equipment, and underneath the prep line.
