Several types of meat items were left on the counter and in the back to defrost at room temperature., These include raw hamburger in the back area and a couple packages of deli meat.
Damp towels were used to cover formed pizza dough in the cooler. , The use of these towels is not allowed as they can pose a contamination hazard.
Back storage area had an accumulation of dust etc., Dough mixer had a build up of flour/oil in hard-to-reach areas on outside of machine.
