January 27, 2026: The facility was found without sanitizer. PHI spoke with the owner, Mohammad, who confirmed that sanitizer is expected to arrive that evening. Meanwhile, the PHI directed the operator to use the on-site bleach according to the manufacturer’s instructions until the sanitizer becomes available., , Jan 23, 2026: No sanitizer was available on site. The operator informed PHI that the Quat sanitizer has been ordered and is expected to arrive this evening. In the meantime, PHI instructed the operator to use the bleach already available on site, following the manufacturer’s instructions, until the sanitizer is available.
The handwashing sink was observed with an empty food container. PHI instructed the operator that the handwashing sink must remain unobstructed at all times and be used only for handwashing purposes.

Staff were observed practicing improper hand hygiene. The operator used a cell phone and then proceeded directly to the kitchen, handling food without washing hands. PHI provided on-site education on proper handwashing practices.

** - the large cooking kettle exhibited a thick build-up of grease and grime on the exterior. , ** - A white container labeled "utensils" was very dirty and exhibited a buildup of food and grime, ** - the panini press was filthy: a thick buildup of grease and grime were noted on the exterior and the interior of the machine. It was hot and in use., ** - some of the cutting boards were overworn and exhibited a buildup of grime demonstrating a lack of thorough cleaning., ** - the shelving in the back kitchen was not clean: spills and debris were observed.
** - one plastic lid was observed to have large spider cracks and missing plastic - discard. Use only food equipment in good working condition that will not potentially contaminate foods with broken parts.
** - beverage cups were observed in both bathrooms. Cease and desist the storage of beverage containers in washrooms., **- The ice scoop was stored in an unsanitary manner on a shelf in the dust. Grime and handprints were observed on the scoop itself. Retrain staff to wash hands before scooping ice and the ice scoop must be scrubbed and fully cleaned daily, ** - the dirty ladder was in contact with clean food utensils. Cease and desist ladder storage in this manner.
** - Poor sanitation was observed within public washrooms: a buildup of biomaterial, urine, dirty liquid spills, grime, and filth was observed on the toilet bases, the sinks, the doors, the door edges, all over the walls, the dispensers, light switches, etc. NOTE: DAILY and thorough cleaning is required. The buildup indicates a lack of regular cleaning., ** - baby change table exhibited a build up of dust and debris within - requires DAILY cleaning and disinfection, ** - toilet paper was stored on the tampon/diaper garbage and not in the dispenser as required.
** - Sanitation was poor with a buildup of spills, grime, grease, and handprints on the following areas that includes but is not limited to: the walls in the hallways leading to the washrooms and the door into the kitchen area had handprints and liquid spills were observed throughout. The wall under the take out pass through to hold tickets was greasy. The front service counter was extremely greasy. PHI took a paper towel and degreaser - wiped a spot - the paper towel was very dirty demonstrating a lack of proper cleaning. The exterior of the canopy was fuzzy with dust. The floors under equipment and in hard-to-reach areas had thick a buildup of grime. The hand sanitizer at the front order counter was very dirty.
