The ceiling tiles were missing above the storage shelve beside the back office., , -Repair/ Replace ceiling tile

Scoops with handle were stored directly in contact with food in the dry bulk food at the kitchen. Operator adjusted scoops., , -Ensure scoops with handles are stored sticking out of the food for easy handling without contamination

Cooler door handles were dirty with some debris.

The following areas had dirt/food debris/ grease build up at the kitchen:, 1. Vent hood over the oven , 2. inside and handle of proofer, 3. the carts in the proofer, 4. inside and handles of the stand-up cooler and freezer, 5. inside of the chest freezer, , -Clean the indicated areas

Bulk food in bags (flour) was left opened at the kitchen., , - Ensure open food are protected from contamination by storing in a lid tight container
