Broken wall tile behind pizza cutting table., Holes in wall / unfinished surfaces (near ceiling) on the outside of the walk-in cooler., Lower shelving in front area is chipped / damaged and no longer smooth and easily cleanable., ALL SURFACES must be maintained in good repair., Fix / repair as necessary., **Outstanding violation - this needs to be corrected**

The floor under the shelves in the walk-in cooler was dirty with food/debris, etc., -clean the floor, wall, shelves, etc in the walk-in cooler. Move shelves to thoroughly clean. This must be cleaned on a regular basis.
